      **A supervisor will return your page or call within 5 minutes and personally take over the 
         call-out procedure. Response time to your company location or emergency room 
         facility will be no greater than 30 minutes from call back (60 minutes to W. Cal-Cam 
         Hospital)

2.) Give EDT representative the following information over the phone:
         -Company requesting drug/alcohol testing services.
         -Patient/ employee name
         -Type of service requested :            Drug screen:   Yes or No (DOT or NDOT)
                                                                       Breath Alcohol test Yes or No (DOT or NDOT)

3.) EDT represrntative will notify employer of any positive test.
         -If patient/employee is unable or unwilling to provide adequate specimen amount for
           drug screen or breath/alcohol tests, then a company representative will be notified
           immediately .
